The review point is part of the workflow
Review should not be a vague promise to check things later. It is a defined step with an owner, enough context to make a decision, and a clear way to stop or correct the action.
Make exceptions useful
An exception queue is a learning surface. Group recurring causes, note what the operator changed, and use that record to narrow the workflow or improve the source data before expanding its reach.
Keep accountability legible
Teams move faster when they know who can approve a change and who can pause it. Make those responsibilities visible in the operating note rather than relying on a hidden default or a shared inbox.
